Agrace HospiceCare partners with a variety of local organizations to support caregivers and receivers all over south central Wisconsin. Please see below for a comprehensive list of local agencies that are here to support you.

AARP Wisconsin
AARP is a nonprofit, nonpartisan organization with a membership that helps people age 50 and over have independence, choice and control in ways that are beneficial and affordable to them and society as a whole, ways that help people 50 and over improve their lives.

Alzheimer’s & Dementia Alliance of Wisconsin
The Alzheimer’s & Dementia Alliance of Wisconsin is a nonprofit organization that is specifically designed to provide a link to resources for people with Alzheimer’s disease or related dementias. We are also deeply concerned with providing support to those who care for persons with dementia. Our mission is to help caregivers take care of themselves as well as provide a quality life for the person with Alzheimer’s disease or a related dementia.

Area Agency on Aging of Dane County
The National Family Caregiver Support program, administered through the Area Agency on Aging of Dane County, is designed to provide information, support and assistance to:

  • Family and friends who help care for a person age 60 and older.
  • Family and friends who help care for a person with Alzheimer’s disease or other dementia regardless of age.
  • Grandparents or relative caregivers age 55+ who are the primary caregiver for their relative’s children age 19 or younger, or who are caring for someone with a severe disability who is 19 to 59 years of age.

Care Wisconsin
Care Wisconsin is a nonprofit care management organization specializing in the integration of health and long-term care services. For over 35 years, we have been filling gaps in community services and providing innovative solutions for seniors and adults with developmental and/or physical disabilities. Today, we manage two of the State’s public health and long-term care programs: Family Care and Partnership. Care Wisconsin serves over 4,300 members across 10 counties in the south central region, and has 500+ full- and part-time employees.

Catholic Charities
Catholic Charities of Madison, WI is a private 501(c)(3) non-profit agency affiliated withCatholic Charities USA, the nation’s largest private network of people helping people. Our organization has served individuals and families in an eleven county area in south central Wisconsin since 1946. The programs we have reach those who are facing the transitions of aging, living with a disability, dealing with a crisis pregnancy, seeking to adopt, having marital or family problems, live in rural communities or seeking help with alcohol or drug abuse. We serve and employ persons regardless of religious, ethnic, racial or social background.

Dane County Caregiver Alliance
Dane County Caregiver Alliance is a network of local professional organizations serving older adults and their caregivers, committed to the needs and interests of family caregivers. The Caregiver Alliance started in 2002 as a result of the enactment of the National Family Caregiver Support Program, through the Administration on Aging. One of the requirements of the Program was to bring together community agencies that serve older adults and their caregivers. The original group of 10 founding members was called the Caring for the Caregiver Task Force. In 2005, we changed our name to the Dane County Caregiver Alliance, which has grown over the years to a group of 30+ members.

Dane County Case Management
Fifteen agencies located throughout Dane County have primary responsibility for programs and services for older adults (age 60 and over). Serving designated geographic areas, these focal point agencies provide the entry points through which older persons – and their families – access a broad array of programs and services, including nutrition and case management. Independent Living, Inc.’s mission is to support adults as they manage their independence by providing a variety of care services, skilled home health care and housing options to maintain health and well-being. We are a locally-based, not-for-profit and have served the community for almost 40 years. We understand, as life progresses or changes, so do your individual needs. National Alliance for Caregiving
Established in 1996, The National Alliance for Caregiving is a non-profit coalition of national organizations focusing on issues of family caregiving. Alliance members include grassroots organizations, professional associations, service organizations, disease-specific organizations, a government agency, and corporations. Recognizing that family caregivers provide important societal and financial contributions toward maintaining the well-being of those they care for, the Alliance’s mission is to be the objective national resource on family caregiving with the goal of improving the quality of life for families and care recipients.

National Family Caregivers Association 
The National Family Caregivers Association educates, supports, empowers and speaks up for the more than 65 million Americans who care for loved ones with a chronic illness or disability or the frailties of old age. NFCA reaches across the boundaries of diagnoses, relationships and life stages to help transform family caregivers’ lives by removing barriers to health and well being.

Supporting Active Independent Lives
Supporting Active Independent Lives is a vibrant non-profit membership organization of people 55+ who reside in the lovely Madison area. SAIL members wish to remain active, independent, in their own homes, and socially connected, and have found that navigating the many changes brought on by age is much easier with the resources offered by SAIL.

United Way of Dane County Caregiver Connection

The Wisconsin Council of the Blind & Visually Impaired, located in Madison, WI, is a private, not-for-profit community-based organization. Our mission is to promote the dignity and independence of the people in Wisconsin who are blind and visually impaired by providing services, advocating legislation and educating the general public.

Wisconsin Department of Veterans Affairs

Located in every county and tribe in the state, Wisconsin’s Family Caregiver Support Programs provide information and assistance to help people better care for their loved ones – and themselves. Whether arranging for services (e.g., respite care, transportation, etc) or helping families understand their options for care, Family Caregiver Support Programs give caregivers access to the information and services that can help them in the often rewarding – sometimes challenging – role of caregiver.

Wisconsin Parkinson Association
The goals of the Wisconsin Parkinson Association are as follows:

  • To provide for both the education and training of physicians and allied health professionals needed to treat the growing number of people with Parkinson disease
  • To provide outreach for education and support
  • To foster awareness and understanding of Parkinson disease
  • To serve as an educational and referral resource for health care providers through out the United States
