How Is Agrace Different ?

Experience: Agrace has cared for people at the end of life since 1978, when “hospice” was a new concept in American health care. Our care teams are highly experienced in all aspects of compassionate end-of-life care.

Facilities and choices:  Agrace’s state-of-the-art Anderson Inpatient Unit and the beautiful, homelike Johnson Residence in Madison give our hospice patients and their families more options, especially when remaining at home for care isn’t possible.

Partnerships: Agrace works with more than 200 area long-term care providers to deliver hospice services to patients residing in assisted living, nursing homes, adult-family homes or senior apartments. Working together allows most residents to remain in the place they call home through the end of life.

Responsiveness: Agrace works to admit new patients the same day that our services are requested, to quickly help bring relief from symptoms and address any other causes of distress.

Access: Patients, families and caregivers can reach an Agrace registered nurse any time of the day or night—whenever symptoms need treatment or questions need immediate answers.

Community support: A nonprofit organization, Agrace has exceptional support from donors and volunteers in our local community, which allows us to expand our services to meet the community’s needs.

Quality: Agrace is a Medicare/Medicaid-certified agency and voluntarily participates in the Community Health Accreditation Program (CHAP). CHAP requires annual self-assessments and on-site surveys to ensure that Agrace meets the latest standards.

Grief support: A wide variety of free, specialized grief support is available to anyone who’s grieving a loss due to death. Hospice counselors offer their expertise both before and after a death to help people survive their loss in a healthy way and return to functional living. Grief groups are held regularly in many local communities, making it easy to find support close by.

Volunteer support: With more than 1,000 active volunteers, Agrace can provide additional help in the home, companionship or simply a caring presence.

More than hospice: Agrace offers palliative care for patients who are not eligible for hospice. Palliative care services range from in-home consultation visits by a nurse practitioner or palliative care physician to Care Navigation services, which assist patients in better managing their chronic or serious illness.

Pediatric palliative care: Caring for children with life-limiting illness takes special attention. Agrace works closely with families, physicians and caregivers to ensure children receive expert clinical care and support through end of life.
